2010 Leadership Scholarship Application

Purpose: The Leadership Scholarship is sponsored by donors of Operation PROM. The scholarship supports students
who are involved in extra curricular activities and/or projects that allow them to volunteer within their community. The
experience of volunteering enables students to grow as leaders, and it also deepens their understandings of leadership
capacities. Through our scholarship program, we aim to invest in students who although have financial difficulties,
realize the importance of giving back to other’s who are in need. We seek applicants who are imaginative and
passionate about their endeavors, and who can envision how they can grow, and contribute through this opportunity.

Criteria: Ideal candidates have made contributions to the improvement of their community through active enrollment in
community based volunteering activities and/or school/organizational groups/clubs (such as neighborhood clean-up,
volunteering at a hospital, involvement in fundraisers, etc…). The recipients will have demonstrated individual initiative,
responsibility, cooperation, and leadership ability. Applicants for the Leadership Scholarship must have a minimum
cumulative GPA of 3.0 and must demonstrate financial need by showing no more than a $30,000 total household
income on their 2009 income taxes. Applicants must be in their senior year at an accredited New York State high school,
a U.S. Citizen, and plan to attend an accredited college/university in 2010.

Award: One (1) student will be awarded with a $1,000 scholarship.
* If recipient chooses to attend the College of Westchester, White Plains, the award will be matched.*

Deadline: A completed application with essay must be received by June 1, 2010.
* When mailing your completed application, please enclose documentation showing your GPA (current report
card/school transcript) and a copy of your household’s 2009 income tax returns. Proof of the applicant’s household
income will be kept confidential. If the applicant is unable to obtain such documentation, an acceptable letter from their
school’s guidance counselor may be submitted. *
